PELVIC INFLAMMATORY DISEASE (PID)

Pelvic inflammatory disease (PID) is a disease of the upper genital tract. It is a spectrum of infection and inflammation of the upper genital tract organs typically involving the uterus (endometrium), fallopian tubes, ovaries, pelvic peritoneum and surrounding structures. It is attributed to the ascending spread of microorganisms from the cervicovaginal canal to the contiguous pelvic structures. The clinical syndrome is not related to pregnancy and surgery.

But, as the terminology fails to pinpoint the precise organ or organisms involved, it is better to use the anatomical terminology in relation to which organ is involved in the infectious process. Thus, the better terminology should be either endometritis, salpingitis, pelvic peritonitis or tuboovarian abscess. The cervicitis is not included in the list. Many, however, prefer the term salpingitis as because it ultimately bears the brunt of acute infection. From a clinical perspective, it is more pragmatic to describe acute salpingitis as sexually transmitted or not.

Despite better understanding of the etiology, pathogenesis, improved diagnostic tools such as sonar or laparoscopy and advent of wide range of antimicrobials, it still constitutes a health hazard both in the developed and more so in the developing countries. The incidence of pelvic infection is on the rise due to the rise in sexually transmitted diseases. The ready availability of contraception together with increased permissive sexual attitude has resulted in increased incidence of sexually transmitted diseases and, correspondingly, acute PID.

The incidence varies from 1–2 per cent per year among sexually active women. About 85 per cent are spontaneous infection in sexually active females of reproductive age. The remaining 15 per cent follow procedures, which favors the organisms to ascend up. Such iatrogenic procedures include endometrial biopsy, uterine curettage, insertion of IUD and hysterosalpingography. Two-thirds are restricted to young women of less than 25 years and the remaining one-third limited among 30 years or older.

Pelvic inflammatory disease is a major problem to the reproductive health of young women. PID may be asymptomatic or subclinical. Currently there is certain changes in the epidemiology of PID. (A) Shift from inpatient PID to outpatient PID. (B) Change in clinical presentation. Less severe disease is commonly seen. (C) Shift in the microbial etiology of more Chlamydia trachomatis than gonococcus and others.

- Irregular and excessive vaginal bleeding is usually due to associated endometritis.

Painkillers Can Cause Erectile Dysfunction

According to a new study, prolonged use of opiate drugs for at least four months increases the chances of men needing treatment for erectile dysfunction in 50%.

The results thus concluded that a regular intake of opioids for an extended period of time might have a negative effect on a man’s ability to have proper erection during sex. But, the patient and especially doctor should be aware of this association while prescribing medicine for back pain.

It is important to reveal that research has found an association, not a cause and effect link. Thus, more research is needed so that this relationship can be understood more fully. Relationships And Ego

Sigmund Freud, the psychoanalyst, says human personality has three parts: id, ego and superego. Each layer has a role in maintaining a healthy relationship.
Ego is your conscious mind, it is responsible for the way you perceive the world, i.e. the internal and external impressions. Id is your subconscious mind: every primitive instinct that you cannot handle or that make you uneasy (aggression, anger, fear) are there in your subconscious mind. This part of your personality demands satisfaction, instant gratification, it wants to experience pleasure and avoid pain. Super-ego is “the ideal self”, the person you want to be, it contains your conscience and values. The layers of your psyche should be in harmony. If there is a huge gap between your id (subconscious mind) and your superego (idealized persona), then you will have self-esteem issues.

Functions of Ego
It has your reality testing ability. It is a skill to perceive what happens within your mind and in the external world. Stress may cause mild, temporary damage to this function.

Impulse control is a skill to deal with your instincts.
Judgment means that you behave responsibly, to measure the consequences of your actions.
Healthy self-esteem means that you feel good about yourself and have self-respect and confidence, even if you face stress.

Freud dealt with psychosexual development. According to him, people have five different stages of psychosexual development. After birth, humans are fixated on their mouths and nursing. The next stage is controlling bowel and bladder functions. After that, they discover their genitalia. The fourth stage is the latency stage, from which they are supposed to grow into the fifth stage: mature sexuality. If the development process breaks at some stage, it will cause a fixation.

Phases of Development

According to Erik Erikson, during the first phase of development, one has to trust others. Otherwise they will have trust issues in their relationships. During the second stage, from the age one to three years, they should develop autonomy. If they do not, they will suffer from shame or self-doubt. In the next phase, from 3 to 6 years, they should learn planning skills and development skills, or they will be dependent all their lives. The fourth stage, age 6 to 12, they should experience success, they should be satisfied with what they can accomplish, otherwise they may grow up to be insecure adults and they will experience guilt. At the fifth phase, they should have identity. As young adults, people should feel optimistic – provided they have a healthy intimate relationship.

Your id, ego and superego – each component of your psyche – define how you select your partners and how to deal with them in the long run. When you select a partner, you see your own superego and the partner’s superego. However, in a long-term relationship, you are supposed to deal with the id of both parties.

Diseases And Disorders That Affect the Penis

The penis is a very important organ in the male anatomy, allowing the emission of urine, semen and participates in sexual intercourse. The penis is also the area of ​​the body through which man, from early childhood, takes on the meaning of his male identity.

The penis is a sensitive organ - able to perceive a high range of pleasurable sensations over the whole surface - and strong enough to make possible penetration during intercourse. In severe cases, the disease lasts for years, the plaque hardens and flexibility is reduced. This forces the penis to bend or arc during erection, which can be very painful. Severe cases may require surgical intervention.

Epispadias: It is a rare congenital anomaly - present at birth - where the opening of the urethra is either on the top or sideways of the male organ instead of the tip of the glans. It needs to be treated by surgery.

Erectile Dysfunction: It is the difficulty of maintain a hard erection, and can be a sign of other health problems that caused this sexual complication, as clogged blood vessels or nerve damage caused by diabetes. Cancer of the penis: It is a rare form of cancer that is curable if detected early. The exact cause of penile cancer is unknown; however, there are certain risk factors: the presence of smegma (greasy smelly substance that accumulates under the foreskin where hygiene is not thorough) that causes irritation and inflammation of the penis; smoking; history of genital warts or human papillomavirus (HPV); and age, since most cases occur in men over 50 years. The symptoms of penile cancer consist of bumps, pimples, warts or painless sores on the penis (usually at the top), and in more advanced stages can experience pain, bleeding and abnormal discharge from the penis.
